A below-40 patient came with his friend, holding his right upper limb up across the front of the body with his left hand. He is a worker and rides a Vespa-type motorcycle. A few hours earlier, he fell from his bike and sustained an injury to his right shoulder joint.
There is a wound (an abrasion) on his right shoulder. However, the was a deformity over his right collarbone. I suggested an X-ray, and the film showed a fracture in his right collarbone. Here is the image
It was a pretty bad, displaced fracture. He needed an orthopedic surgeon to fix the fracture with plates and screws (most likely). So, I referred him to the higher center! A collar and cuff sling (arm sling) wasn't enough to fix the issue.
Sometime later, three Omani young men (from the sponsor side) came to my chamber in an angry mood and asked me why I sent him to the higher center instead of just prescribing an arm sling!! Because consulting an orthopedic surgeon would cost a good amount of money! I was able to convince them to send the patient to the higher center! Yay!
